Position 1: Quality Improvement Officer
Purpose of the position:
The Quality Improvement officer is responsible for developing quality improvement aim/objectives and come up with change concepts to generate change ideas to improve the progress and performance of the program in the implementation of Family Focused HIV Prevention, Care and Treatment Services and is responsible to ensure optimum data quality assurance. The officer also provides technical support and advice to project sites staffs and stakeholders on the identification and rectification of all data quality issues associated with the program quality improvement and Progress.
Major duties/Job Summary:
- Identify program problems and develop quality improvement(QI) aim/objectives and address the issues
- Establish QI team, lead and support quality improvement team members
- Generate change concepts and change ideas to improve program performance
- Plan and tests change ideas for scale up
- Facilitate systems analysis, using systems modelling, flow charting and root-cause analysis
- Identify training/orientation needs, develop and implement training/orientation of relevant QI staff at both at the regional office and program sites.
- Responsible to work with technical staffs and develop, maintain, and implement plans to achieve quality improvement aim/objectives/goals.
- Provide technical support to program sites in developing and implementing quality improvement activities in all program areas.
- Oversees and monitors formal quality improvement activities and develops performance improvement targets with in the program.
- Ensures comprehensive data analysis to identify areas for improvement across thematic program units.
- Provide onsite technical assistance to project QI teams and organize all QA activities and work collaboratively with the other teams to address program gaps.
- Support and facilitate the project QI committee meetings and implementation of the project QI plans
- Contribute to the documentation and dissemination of QI related activities, progress, successes, lessons learned, and best practices

Education, Experience and technical skills
- BA in social science, Health science, statistics and other related fields of study.
- Minimum of 3 years work experience in data quality management or Monitoring and evaluation.
- Proven ability to conceptualize, innovate, and execute ideas
- Ability to work independently with strong problem-solving skills including the ability to foresee problems and initiate appropriate action.
- Knowledge of applicable tools, concepts and methodologies of QA and QI
- High level of computer literacy – Advanced level knowledge of Microsoft Excel, working knowledge of other Microsoft software to investigate, present and interpret data.
- Demonstrated experience utilizing Microsoft office software, especially Excel, IT and software system knowledge to manage quality, integrity and validity of data
- Demonstrated high level, professional written and verbal communication skills in order to prepare complex business and technical documents and develop relationships with ongoing stakeholders
- Experience in identification of data quality issues and provision of recommendations for data system and process improvements
- Experience using data to improve program performance and/or supporting facility-based quality improvement teams.
- Service delivery experience, especially in HIV and family health services
- Experience/Knowledge on Current literature of Adult/ Pediatric HIV management, TB, OVC, Prevention and PMTCT services.
Position 2: HIV Service Linkage Officer
Major Roles
- Facilitate linkage between community to health facility and vice versa putting the need and choice of the clients and their families at the center of the required care for linkage (CommCare application will be introduced to enhance referral and linkage service)
- Case management for PLHIV in the community including recording and reporting using paper based system which late migrate to CommCare application
- Build capacity of SSWs and CRPs (volunteers) through providing training’s mentoring and supportive supervision
- Build Capacity of community structure (community care coalition(CCCs) by providing hands on support in planning, implementing and reviewing community based HIV related activities such as identifying beneficiaries for community based support for linkage to relevant services.
- Recording and Reporting of community services: will record and report community based community based care and support activities including electronically through introduction of CommCare application.
Specific Roles and Responsibilities
- Facilitate CHCT’s engagement at community level,
- Engage community members in planning, implementing and reviewing community related activities including linking with the health Extension program
- Work with Health facilities in linking clients to other social services in the community such as pyscho social and economic support (counseling, IGAs…)
- Identify cases that are going to be referred to community from health facility together with the health facility provider or case manager collaborated with heath facility level case manager in identifying stable and unstable clients
- Ensure linkage of clients refer from community to health facility
- Provide community based case management for stable and unstable patients in consultation with the ART centers together with CRPs.
- Support follow up of clients at community level on appointment spacing
- Participate in community based service delivery model as per the MOH’s future guidance to differentiate care to clients need and preference.
- Facilitate /provide community based care and support for non-stable patents to lesson burden to health facilities and clients.
- Assist CRPs in screening of TB,STI, Mental health and other OLs based on the need for such support
- Provide technical assistance to case managers, social service workers and community resources persons
- Training of case managers and CRPs to get them acquainted with task below for social service workers
- Mentoring and supervising CRPs and case managers to get them acquainted with task below for CRPs.
- Provide technical assistance to (CCs) and (CCCs)
- Be part of the CCs/CCCs and provide technical assistance in their routine tasks.
- Participate in beneficiary identification including assessments of household for economic vulnerability and food security
- Ensure volunteers are involved in meaningful way as per the guidance of Cc/CCs
- Conduct index case testing at community level
- Receive list of families of index cases and apply risk assessment for testing
- Identify sextual networks of index cases in confidential and systematic manner
- Counsel high risk family members for testing and refer for health facility
- Conduct assisted ,partner notification and do community based testing based on clients preference
- Facilitate self-test
- OVC care and support and prevention
- Ensure OVCs on comprehensive care provided service based on standard and graduate based on SOP
- Ensure 9-14 years OVCs on prevention package reached with curriculum based prevention training.
- Use and implement COmCare app/mobile app for referral and linkage and case management
- Oversee implementation of CHCT in relevant community based activities including stigma reduction, Gender equality, women empowerment that contribute to improve targeted testing and quality of HIV care and treatment

Qualification and Relevant Work Experience Required
- BSC in Health science minimum of 5 years’ experience in providing HIV treatment, Testing and Counseling Services.
- Specialized Knowledge: Training in Adherence Counseling, Couple Counseling and community HIV counseling. Up to date with latest developments in HIV differentiated Service delivery Models
- Experience on Previous CHCT and CVC programs Highly preferable
- Experience: minimum of 3 Years’ Experience in Providing HIV Testing and Counseling services. Experience in HIV care and support activities preferably at community level.
- Ability to coordinate and support volunteers working in the community
- Good interpersonal and communication skills
- Ability to work effectively in a team and support others in their work
